Supporting Individuals Starts with Supporting Caregivers

At Your Choice Residential, we believe meaningful residential support starts with strong relationships, compassionate caregivers, and a team-based approach. Whether you are an experienced Host Home Provider, a family member supporting a loved one, or someone interested in opening your home to an individual with intellectual and developmental disabilities, we are here to guide and support you every step of the way.


Our team brings experience from direct support, case management, behavioral services, investigations, compliance, and residential leadership. We understand both the day-to-day realities of caregiving and the systems that surround HCBS services in Colorado.

What Is a Host Home Provider?

Host Home Providers (HHPs) offer individualized residential support to adults with intellectual and developmental disabilities within their home environment. 


The goal of IRSS services is not simply supervision. It is helping individuals live meaningful lives with dignity, safety, stability, and opportunities for community involvement and personal growth. 


 Our caregivers assist with a wide range of needs depending on the individual, including: 

  • Daily living skills 
  • Medication management
  • Transportation and appointments 
  • Behavioral and emotional support 
  • Community participation 
  • Household routines 
  • Health and safety monitoring

Many of our caregivers come from healthcare, education, behavioral health, or direct care backgrounds, while others simply have a passion for supporting others and creating a safe, welcoming home environment. As our name entails, we are all about CHOICE and we are always open to expanding our caregiver network in order to be able to offer our members as many options as possible when they are looking for a host home. We believe that while experience is valuable, ensuring a placement is the "right fit" for everyone involved is very important to us and we are happy to work alongside new caregivers as they dive into the world of supporting individuals with disabilities.

Family Caregiver Services

Many individuals receiving HCBS-DD services continue living with parents, siblings, or other family members. We recognize the tremendous amount of work, advocacy, and emotional energy family caregivers provide every day.


Our role is to partner with families by helping navigate waiver systems, provide ongoing support and guidance, and ensure services are implemented in a way that is person-centered, sustainable, and compliant with HCPF and CDPHE requirements. It is important to us that home still feels like home to your family, despite the rules and regulations that these services exist within.


We strive to build collaborative relationships with families while respecting the expertise they already have regarding their loved one’s needs, preferences, routines, and goals.

How to Get Started:

  • Step 1: Complete the Screening Form (linked below)
    • Tell us more about yourself, your experience, and the type of caregiving opportunity you are interested in.
  • Step 2: Connect with Our Team
    • A team member will review your information and discuss potential opportunities, expectations, and next steps.
  • Step 3: Background Checks & Onboarding
    • We will guide you through required paperwork, trainings, and onboarding requirements.
  • Step 4: Matching & Transition Planning
    • When we are looking for placement for an individual, we carefully consider compatibility, support needs, routines, personalities, location and long-term stability as we determine potential options.
  • Step 5: Ongoing Support
    • Our relationship does not end after onboarding; our goal is to help caregivers feel supported rather than isolated. We continue providing support, communication, and guidance throughout residential services and beyond. Colorado waiver systems can feel overwhelming. Our team is available to guide caregivers through system navigation and documentation requirements every step of the way. We collaborate closely with other guardians and family members, CMAs, medical providers, therapists and behavioral health providers, day programs and employment supports, pharmacies, and whoever else may be an important member on your team to ensure team cohesion and the best possible supports for those we serve.
